<body topmargin=0 leftmargin=0 marginheight=0 marginwidth=0> <table cellpadding=0 cellspacing=0 border=0><tr> <td valign="top" colspan=2 height=90 BGCOLOR="#006699" TEXT="#080000" background="04b785a3.jpg"> <div> <FONT SIZE="5" COLOR="#FFFFFF" FACE="Arial" ><B>Healthy Fathering Collaborative</B></FONT><B>    </B><B> |     </B><A HREF="index.htm" TARGET="_top" TITLE="Healthy Fathering Collaborative"><U><B>home</B></U></A></div> <bR> <div> </div> </td> </tr><tr> <td valign="top" width=210 BGCOLOR="#FFCC66" TEXT="#080000" background="04cd2b03.jpg"> <div> <I>Child Support Resolutions</I></div> <bR> <div> <A HREF="id18.htm" TARGET="_top" TITLE="Programs for Fathers"><U><B>back to Programs for Fathers</B></U></A></div> </td> <td valign="top" height=390 BGCOLOR="#FFFFFF" TEXT="#080000"> <bR> <div> <B>Child Support Resolutions          </B><FONT SIZE="2" COLOR="#000000" FACE="Arial" ><B>                                                                                                                 </B></FONT><FONT SIZE="2" COLOR="#000000" FACE="Arial" ><B> </B></FONT><FONT SIZE="2" COLOR="#660000" FACE="Arial" ><A HREF="index.htm" TARGET="_top" TITLE="Healthy Fathering Collaborative"><U><B>home</B></U></A></FONT></div> <bR> <div> <B>Mission:</B>  The purpose of Child Support Resolutions is to provide timely and appropriate services to our clients regarding child support issues.</div> <bR> <div> <B>Goal: </B> The goal of Child Support Resolutions is to correct the issues and dissolve the barriers that hinder a parent from being a participant in the child's life.</div> <bR> <div> <B>Value:</B>  We have the passion to help our clients make the first step towards a responsible future.</div> <bR> <div> <B><U>Services and Fee Schedule</U></B></div> <bR> <div> <B>Initial Intake - </B>Gathering information required.</div> <div> 1-2 hours</div> <bR> <div> <B>Paternity testing</B> - Signed affidavit, genetic testing, all information on establishment of paternity orders, as well as all information that pertains to you, will be discussed.</div> <div> 3-5 hours</div> <bR> <div> <B>Wage attachments</B> - Required information is given to Child Support Enforcement Agency within 1-10 days of hire.</div> <div> 1 hour</div> <bR> <div> <B>Retrieval of Drivers License</B> - First suspension includes lump sum payment based upon percentage of the arrears.  Second suspension includes lump sum payment of 50% of arrears and employer is required.  Third suspension includes full payment of arrears.</div> <div> *Billable hours dependent upon number of past suspensions.* </div> <div> First suspension 1-2 hours (based upon no calculation needed).</div> <div> Second suspension 1-2 hours (based upon no calculation needed).</div> <div> Third suspension 1-2 hours (based upon state mandated requirements).</div> <bR> <div> <B>Calculation</B> - Calculation based upon last established court order to current date, calculation may also be based upon last order on arrears to current date, emancipation of child (if more that one on order), degree of difficulty of a calculation will also be based upon number of past Child support systems used to present, welfare involvement, retrieval of payment information.</div> <div> *Billable hours based upon degree of difficulty which will be discussed upon retrieval of information from client*</div> <div> 5-18 hours</div> <bR> <div> <B>Retrieval of order from Clerk of Courts</B> - Certified copies are always required for all actions (these are at a cost to the client). </div> <div> *Billable hours based upon location of court, or based upon degree of difficulty which will be discussed upon retrieval of information from client*</div> <bR> <div> <B>Emancipation of child</B> -Work and billable hours are based upon cooperation of all parties involved, as well as, degree of difficulty, which will be discussed upon retrieval of information from client.  Calculation is required before any order would be reduced or terminated.  </div> <div> Please refer to calculation of billable information.</div> <bR> <div> <B>Modification</B> - State requirements include 30% change of circumstance, or three years.  Proof would be required why modification requested, making it an issue before it becomes problem.  Modification request once given to agency has 60 days to make a determination, if modification is accepted, follow through for all steps up to, but not including possible court action. </div> <bR> <div> 10-15 hours based upon degree of difficulty of retrieval of information from all parties.</div> <bR> <div> <B>Agreed Journal Entry</B> - This action can only be done 1 time before the Child Support Enforcement Agency for direct payment to client;
